Whooping cough cases increase

pills Please use the right hand side of your prescription and tick the items required. If you have misplaced your repeat form please write down your request and send it in to the surgery.

Your prescription can either be put into the Lloyds in house chemist or, by arrangement, be collected and returned to a local chemist whichever is more convenient.

You will be able to collect your prescription within two working days. If you wish to have your prescription sent to you please supply a stamped address envelope.

We are unable to take prescription requests over the phone .
